Scroll Portal to town bug

This has occurred a couple of times now to me and my roommate.

Cast town portal and die as soon as the town portal appears (don't think the animation has finished)

Appear in town. The portal that should exist in town is not there.
Waypoint back to location I died in and find the portal there still up.

Cast a new town portal and take it to town. In town there is no portal showing nor is the graphic just missing mousing around the area does not give a portal name highlight.

The only fix for this I found was relogging. This has accurred in Cruel in different acts for me and my roommate. Mine occurred in Act 3, His in Act 2 I believe. I was playing in Domination league when it occurred he was playing in Standard League.

I don't believe gear or character to be a factor at all due to the disparity in play style and gear between us. The common point is us dying either in the middle of casting or as soon as it finishs (personally I believe the deaths are occuring during the casting but the animation forces finish which creates the ghost town portal )

no instance did NOT reset.

Mine was Act 3 cruel Marketplace.
