Failed to Join Instance Because Disconnected from server

Happened to me several times too in maps with fracturing mod and in The Coward's Trial.

This issue has happened to me twice over the span of two days, in pretty much exactly the same manner that OP has listed. I have hence lost two maps due to this issue. I was running a summoner in the Dungeon Map.
Is there a fix for this issue yet or what? Mods on Dungeon Map: 12% Quality 2 Extra rogue elites? (unsure about this mod) 27% extra monster life? Log file data listed below just before the crash

Freaking thing happened again, mods: Fracturing, 2 exile rogues, temp chains, and sth else.
Went to city to sell stuff, then got disconnected while entering the portal again, and after this all other portals broke with Failed to Join Instance Because Disconnected from server. Fix this damn thing already.

Just happened to me on a Deadly Dunes of Hordes. It happened when I tried to use the 3rd portal or so to the map.
If this is of any relevance, when first entering the map, I experienced huge lag spikes (1000+ ms latency) but they toned down after a while. just happened to me on a Dunes map with 114% quantity.
fracture reflect 18% elemental cursed with enfeeble gain 3 frenzy charges every x sec area has 2 unique bosses entered in cleared first area went to sale and disconnected when i tried to reenter got disconnected. when i relogged i got "error: failed to join instance because disconnected from server. error: Failed to join any instances". i know the instance wasnt closed cause i just made it 5 min before and spent less then 1 min in town. this really pisses me off... | |

This just happened to me - fracturing with 2 exiles.
This is very frustrating. Happened probably 4 times in the last week of running maps...... Map drops are already low, but when you do get the one you want and invest some hard earned currency. I would imagine this is detectable, even a partial refund would be great. I've heard if you don't Portal in an area with corpses that it's fine in fracturing maps, but the crashes are related to portals near corpses.
I haven't personally been able to verify, but it might be something that can save your fracturing maps. 90% of people put portals in areas they just completed kills and looting to go back and sell. Try running to a "Clean" area (like the entrance) and portalling and see if it works better. | |

I had another sudden DC from map (with facturing mod) today.
