If you buy a microtransactions lets say Skull hood you can you use it once or more times?

Yes. After you apply it to an item (click 'M' in-game to access the microtransaction menu, right click the effect and left click the item you want to apply it to) you can reclaim the effect (click 'M' and look at the top right corner of the menu, where it says "Reclaim Effect", click that and then click the item the effect is currently on).
