Stuck in Act 2 Cruel before Ruthless portal

I have all the quest done, I beat the act 2 boss in cruel, but I am unable to use the door to progress. I don't even have the key to activate the boss fight anymore. Do I have to redo all the bandit bosses to the the combined amulet again?

Bandits don't drop amulet pieces anymore it seems, so I guess I am stuck in Cruel...

I had this issue going from normal to cruel. I had someone create a portal in the next difficulties Twilight Strand.
