Some Quests won't finish or quest items won't drop

On my Templar I've had several quests items not drop when I kill the appropriate boss.
On Ruthless getting the spear from Weaver, I tried several times killing the boss and resetting the instance and the spear would not drop. I eventually had to group with a friend for it to drop. Same thing happened with the baleful gem. And now in merciless killing the Lurker in the flooded depths, it won't credit me with the kill or completing the quest. Is this a known issue with a fix? |

This sounds very unusual, would you mind mailing me a screenshot to rory@grindinggear.com of you having killed the weaver and the spear not dropping, with your quest log open on that quest? It might be something about the location and position of the boss that is preventing the spear from spawning.
As for the Lurker, that sounds like it shouldn't be possible. What skills were you using to kill the boss? Were degeneration effects like Viper Strike, Burning+Elemental Proliferation, or minions involved? Balance & Design
