"Disconnected: Failed to connect to instance"

This is the error message that I receive after getting kicked back to main menu. I get this 98% of the time when I attempt to pass through any loading screen or zone change, or even logging into lioneye's watch where my level 7 character has been stuck since league start since I can never make it past mud flats without getting this error. I have tried all advised solutions I could find from people with similar problems and nothing works. Literally unplayable. Please help.
