Another "failed to join instance"...

I just randomly put up a town portal and I get this error message saying something like "failed to join instance..."

How long has this issue been going on because as far back as I can remember it's been a thing. Please fix... getting really tired of losing progress because I can't leave a map when I need to and then come back to finish up
