Need some advice with my build, please!!

This is my current skill tree distribution:

I have two unallocated points and I'm willing to buy regrets to rework my tree if necessary.

My current stats are:

Health: 2988

Armor: 2297.
With determination gem: 3400. With determination and a 17.6 second molten
shell: 5007

Resistance:
Fire: 80%
Lightning: 33%
Cold: 32%
Chaos: -60%

DPS:
Heavy Strike: 5569.4 Gems: melee physical damage, added fire damage, and faster attack.
Leap Slam: 1040.2 Gems: life gain on hit, and reduced mana.

Anyway, I seem to die pretty fast and not just to monsters that deal chaos damage. I'm wondering what I can do different to improve my characters survivability so that I can play solo with some confidence. Do I just need better gear? Should I change my skill tree around? Should I change my choice of gems? Any and all advice will be greatly appreciated. Reworked my skill tree a bit to maximize passive skill placement and utility. I now have 5 extra skill points... Debating where to place them.
