Beyond Sight III (Prophecy)

I have this prophecy, and I believe it is supposed to trigger in the Chamber of Sins level 2.
I have visited several times in both Act2 and 7 without triggering the chain. I have even cleared both zones in case it triggers from a mob. Needless to say I am frustrated! Is this a bug? or am I in the wrong place? Last bumped on Sep 16, 2018, 7:39:30 AM
