Elder kill

Just killed t15 elder, Zana didnt appear. Elder didnt had his second phase, he just died and didnt drop any of his loot.

Same just happened to me in T15 Basilica. And it still says "defeat the Elder in Red maps" and 14/15 fragments... Why is that, is it a bug or something i missed (shaper survived)
