This was a corrupted Vault map. There were many rares. The map is fully cleared, but the prophecy did not complete. Bug reference number 2044037162.

I believe this has to do with incursions; if an incursion spawns, a rare monster in it is twinned, but you don't kill them, it does not count towards the completion of the prophecy.
