Instance crash upon zone re-entry

As the title suggests, I've had multiple instance crashes the last couple of days while leveling. If I enter a master mission or vaal "side-area", then return to regular area I would disconnect instantly. After logging back in the area/instance and any corresponding portals would disappear. (Also can't access the "crashed" instance using 'ctrl + click' method from the previous zone.)

It does not seem to happen every time, but it has happened at least 4-5 times in the last couple of days. Before this weekend I never had this happen - ever that I recall.

Edit 1: Regular leaving a zone or logging out does not cause a crash on re-entry

Edit 2: Just happened in graveyard map with no side area. Just left to sell and disconnected/crashed when i went to re-enter.

Since zone's are server side, I assume it has nothing to do with my pc. Let me know if you need any details on my pc onfig.
