The Eradicator Bug (Two different cases)

Earlier today I tried doing The Eradicator, I died at his second phase and entered back in and he was gone. Only a some of his tentacles remained, and the lightning ball in the middle. Boss was gone, and did not reappear after killing his tentacles.
https://imgur.com/eHP0MqS Later on, I tried again and got an instance crash and disconnected when (I suspect) he was within Slayer cull range. Hey got the same Problem here. Died at the second phase and after i got back he didnt show up again.

Had the same problem - Eradicator was gone after I died and went back (during first phase), no way to proceed.

Same for me. Eadricator was gone after i died and came back. Can't progress!

I can't enter the boss arena spires of delusion, it crashes my game during the loading screen

I have a bug too with this guardian...
I was in excavation map when i do it....I did the first phase(i guess) he was in more than half health done..and he vanished and left me with only some sparks going on...what should i do? | |

I also got the loading screen bug. I fought boss for a while, then i died. When i try to return to eradicator's location, game repeatedly crashes. Second time I completed the map and got to the boss fight, game crashed after few seconds of boss fight without me even dying. If there's a way (in the settings for example) to fix it, please help.
