Unable to use portals after summoned with Portal + Cast on Death

Recently started using Portal + Cast on Death level 1 for no reasons in particular. After this was activated in a map (some T6's, T10's, and Actions Nightmare) in groups and solo play, no party members were able to use a portal to leave. Even after each party member had tried scrolls. Only way to leave was back to character select screen.
