Couldn't pick up item

Unfortunatley, i forgot to type debug :(
http://postimage.org/image/t6j9f0box/ I couldn't pick up this multiple projectiles no matter how hard i tried. I tried from both sides of chest. I also went down below thinking it might dropped down, nope, no luck. This is the 1st time i had this kinda of issue. It happaned in chaos. Insan

I've had this happen a couple of times and didn't know there was a debug command. For future reference how do I use debug?

Sadly, we can't fix the problem without the debug info in the screenshot!
Typing /debug into chat will give us information that we need in the screenshot, that'll let us know where this happens, and that'll let us prevent it happening again. Balance & Design

This probably won't be of any use as I was unaware of the debugging info protocol when it happened and did not collect the info, nor screenshot.
I played for a few hours for the first time on Aug 26, up to level 9. I opened one of the larger chests and out popped a Scroll of Wisdom and a Town Portal. Both items dropped right in front of the chest and the lid came all the way open on top of them. No matter what I tried, I could not pick them up - it was like they'd disappeared.
