i keep crashing every time im go to a new zone it make me crash 10-15 time already pls fix this bug i cant take it anymore

I'm having the exact same issue. I crash 90% of the time I use a portal. Extremely frustrating especially playing Low Life.

These are the two crashes occurring gathered from Windows logs
These crashes both occur with DX9 and DX11 as I've tried both trying to fix the issue.
It also always happens on the loading screen like this http://imgur.com/a/DEQk7

Use the steam version, disable VSync, anti-aliasing, & multi-threading.
