Account logged in from a different location!!! Clarification.

I got a mail saying my account was not logged from my usual location and had to unlock it via unlock code from my mail


Is this possible even if someone actually didn't login in to my account and some IP thing caused this to happen or what are the other possibilities for this to occur.
