Same issue here. Random LA build. Been screwing me in maps for the past 4 or five days.

I have had this issue once a while for some time.
The symptoms were a little different though: I was unable to return from a map to HO; today I was unable to enter last Aspirant's Trial and later on any new instance from Sarn. I saw what Hege wrote and did some testing. It is definitely caused by Bone Offering in my case. Removing the gem and swapping weapons back and forth helps. Sometimes removing the gem was enough. There is no dark, it's just absence of light.
