Lag Spikes + Disconnects

WinMTR stats below. Have been having issues for about a month now. Have tried a number of options to workaround/remedy but to no avail.

The lag spikes seem to occur for 1-5 second time periods making it near impossible/impossible to play HC. Happy to provide additional information if needed.
