I got disconnected by the server for no reason what so ever (my ping is constent 40 and i don't have connexion issues). I reconnected 5sec later and my apex portals were gone. Did anyone ever had the same problem?

Your character should still be flagged regardless of the items existence (as long as you had it completed).
Just go to the temple where you're supposed to use it as if you still had it and it should work. | |

that's the thing when i came back to my hidehout thinking it would have only used one portal out of the 6, there were just no portals at all.

Oh I'm sorry, I got confused with what you meant.
I thought you meant the triangle object that you use to release the Vaal Oversoul. As far as instances go, it sounds like the instance itself crashed or was reset which would invalidate all the portals inside. Unfortunately there is no way to get them back without a new map.
