No.

You hit one target. You deal some amount of damage to it, some portion of which is physical damage.
You leech based on that physical damage. If you have Vaal Pact this will be instant, otherwise it adds an over-time leech effect to you.
You take reflected damage from the hit.
Now the skill might hit another target, repeating the above steps.
