Vsync not working?

I am having an issue where my fps is switching between 250, 333, 500, and 1000, but my monitor is 60 Hz and I have vsync on.

Graphics card is a GTX 760.

Vsync enabled where? In-game? You might have it "off" on NVIDIA Control Panel if so.

are you playing windowed or fullscreen?
vsync doesn't work in windowed mode. |
