Ammulet for SA Phy/crit

Looking for an amulet with a maximum buyout of 5ex (not 6 or 7 but 5)

Here is a list of desired mods in order of importance.

#1. Critical Dmg %
#2. Good HP
#3. Life Leech
#4. 10-15 All Res (or just fire/cold)
#5. Critical Chance %
#6. Raw Physical Dmg

Top Three are a must.
Fourth is highly Desired.
Fifth and Sixth are added bonuses.

Anything else has zero value to me.

Prefix and suffix still open. Has all three required mods. Can mastercraft 10 all resist.
HP roll is only fair, but has top crit multi and leech rolls if you don't count legacy.
