played a good amount in ambush, never saw one, leveling my 3rd character in Rampage I stumbled on one in a corrupted zone, the kaom's one. Axe dropped, I was sad, the end.

I chanced a koams box first week of rampage, haven't tried before or since. I didn't even know unique boxes existed, just happened to have a chance orb and no trans with me.
