Terrible framerates especially when in windowed

I just got back to the game having not played for a year or so, now with some updated hardware and downgraded performance. Launching it for the first time it goes between 5 and 25 fps, more or less regardless of settings. Setting it to fullscreen makes it go to 50-70 with vsync on and 120 ish with it off, however whenever there's stuff going on it drops down to 5 or so again momentarily and is very very bouncy.
CPU: AMD FX-8350 GPU: AMD R9 290X / HD6450 RAM: 16GB 64-bit windows 7 Catalyst 14.92b currently No real issues in any other games or benchmarks. Tried various different graphics driver versions with no change. In game options change nothing. Reinstalled PoE on both a mechanical drive and an ssd. Tried setting affinity to a single core and tried -nosound. Same results.

Same problems
1st beach Loc - FPS is 100+ (act 1 normal) 2nd beach loc - FPS is 15-30 (act 2 cruel) Whats wrong with this game? Usaly Locs with Rain are droping FPS for 50-80% WE NEED WEATHER - OFF - option at Graf Settings |

Right-click the desktop -> Personalize -> Scroll down and select a basic theme to disable Aero
OR Right-click the PoE shortcut -> Properties -> Compatibility tab -> Disable Desktop Composition There are probably some tricks to your AMD driver settings which will increase performance too, but I don't know them off-hand, just try to set everything to Application Preference if it's an option. My game was choppy running in windowed full screen too. I fixed it by unchecking the "Use Windows XP Style DPI Scaling" box that I found here: Control Panel > Hardware and Sound > Display > Set Custom Text Size (DPI).

Tried the standalone version and it works slightly better, the difference between windowed and fullscreen is basically gone. Still get the dips constantly tho. This is with all the suggestions mentioned previously in the thread applied. Also played around with some different install locations, currently best working is the standalone on an ssd.
Not sure if its related, but the loading time when first starting the game seems very long. Dont really have any reference here so could of course be quite normal.
