Terrible frame time peaks make game unplayable.

Sorry to hear you're having this trouble. One option we have found that may help is that some players with NVidia cards have achieved better fps by turning on the vsync setting and toggling threaded optimization in the NVidia Program settings:

Opening up Nvidia Control Panel and heading to 3D Settings -> Manage 3D Settings -> Program Settings. Change the program to Path of Exile, and then scroll down to the vsync setting. Change this setting to On. Please also change the 'Threaded optimization' setting to Off.

Apply these changes and restart your computer, and let us know how that goes for you. It may also help to try toggling the post processing option, and making sure screen shake is turned off. Thanks
