Failed to connect to instance

I am getting a failed to connect to instance disconnect extremely often. Sometimes it's the first area I go to, some times its the second or the third. As a summoner this is EXTREMELY frustrating as I have to go get specters again and again making this game totally unplayable.

I have disabled all adblock and windows firewall and it still persists. It just started happening like a day ago and was not associated with a patch as far as I can tell, which just makes me more confused.

My internet connection is mostly stable but does occasionally hiccup. The hiccups are few and very far between. It does seem to effect my internet though. like 20-25 seconds after a PoE disconnect, a twitch stream will pause for a bit and then go again. I might also not be able to open a new tab for about 10 seconds. I will however not disconnect from anything else and I will immediately be able to log on to my works e-mail program like there was no problem even if twitch is not playing and a new tab is failing to load. If I don't log into PoE all day, I would not notice any mini interruptions, but playing PoE seems to mess it up a bit.

I don't know if its related to this other minor problem I have, but when I am changing areas, sometimes PoE "Stops responding" for like 3 seconds and I get the little circular loading icon on windows and a (not responding) next to the name at the top. It usually picks right up after 3 seconds and loads the zone though. This has been going on for months though.

It is very possible it is a problem with my ISP. If it is, let me know and I will give them a call.

Here is the WinMTR. I'm not a network guy, but it doesn't look like I'm losing a ton of packets.

Thanks.
Have the same problem....
This looks like a problem with the hardware being limited somehow. Perhaps it's trying to load resources from a very slow disk, or is processing graphics with a buggy driver. In any case, without more information it seems like all we can rule out is the network being the problem when you tested it (are you on wifi?).

Please run a dxdiag and Spoiler the report here. If it is too long, leave off the end until it fits.
